Video and Audio Consent Form
Tuesday, October 7th, 2008To download a video and audio consent form as a Word document for use when producing media. Instructions:
- You must have Microsoft Word 97-2003 or higher on your computer
- Left click on Download next to the Microsoft Word icon directly below this text box.
- You will be directed to a page which has a large green button in the top center which says Download.
- Left click on the large green button to Download the Microsoft Word file.
- The word document containing the sign up sheet will open momentarily. Examine it.
- Save the file to your computer for printing and future reference
